The vacancy list of Health Assistants (Ward/Union/Upazila wise) will be available on the notice board of the Office of Civil Surgeon, Lakshmipur, and all Upazila Health and Family Planning Officers under its jurisdiction and on the website (http://cslakshmipur.teletalk.com.bd). Interested candidates can fill out the application form at https://cs.lakshmipur.gov.bd website. The form should be filled out as per the instructions published on the website. The date and time of online application filling and examination fee submission is 22/04/2014 AD from 10:00 AM onwards. The last date and time for submission of the online application is 14/05/2024 AD till 5:00 PM. Candidates who have received a User ID within the said period can submit the exam fee through SMS within 72 (seventy-two) hours from the time of online application submission. In the online application form, the candidate has to scan his colored photograph (length 300 pixels x width 300 pixels) and signature (length 300 pixels x width 80 pixels) and upload it at the designated place.
